Child Under Age 16
Step 1 - Prepare Passport Application Documents
A) DS-11: Government Passport Application
DS-11 must be completed online using this government application form.
- Do not sign this form now; you will sign it in front of the Acceptance Agent (AA).
- You must print out 2 copies of the DS-11 form
- Leave 1 copy on the OUTSIDE of the sealed envelope.
B) One Passport Photo
The photo must be 2×2, color with a plain light or white background, and show a full front view of the face.
C) Proof of US Citizenship
Proof of citizenship must be an original or certified copy. The original document will be returned to you with your completed passport. No photocopies or notarized copies are accepted. Please provide one of the following:
- Official Birth Certificate (if born in the US), OR
- Most Recent Passport, OR
- Naturalization Certificate (for US citizens through naturalization), OR
- Certificate of Citizenship (if born abroad to US parents)
D) Evidence of the Child’s Relationship with Parents or Guardians
As evidence of the child’s relationship to parent or guardian please provide an official copy of one of the following:
- Official U.S. Birth Certificate (with parents’ names)
- Foreign Birth Certificate (with parents’ names and official translation)
- FS-240 or DS-1350 (with parents’ names)
- Adoption Decree (with adopting parents’ names)
- Court Order Establishing Custody
- Court Order Establishing Guardianship
Note: If the parent or guardian’s name has changed please send official proof of that name change.
E) DS-3053: Statement of Consent (if necessary)
Both parents must appear at the AA to sign the application for a passport. If one parent cannot be present when visiting the AA, then the non-appearing parent must sign the statement of consent authorizing the passport issuance for the child.
- The statement of consent must be Notarized.
- A photocopy of the non-appearing parent’s valid driver’s license or valid passport must be included.
F) Proof of Parent’s Identity
The parents need to bring proof of Identity in the form of:
- Valid Drivers License
- Valid Official U.S. Military ID
- Valid U.S. Gov’t ID
- Valid U.S. or Foreign Passport
G) Letter of Authorization
Download the Letter of Authorization, fill it out, sign then print 2 copies of this form authorizing DC DAATS to handle your passport service request.
H) Travel Evidence
Please provide two copies of one of the following:
- Copy of plane ticket showing foreign departure, OR
- Copy of travel itinerary (from airline or travel agency with applicant’s name, travel date, and flight info)
I) Government Fee & Expedite Mailing Fee

Step 2 - Fill Out the DCDAATS Online Order Form
All applicants must complete and submit the Online Order Form. After submitting the order form please print a copy of the “confirmation page” and send it to us along with the rest of the documents listed above.

Step 3 - Get Application Authorized
The parent(s) or legal guardian(s) of the child must appear in person, with the child, at a local court house or post office and present the list of items in step 1 above to a passport acceptance agent (AA). The AA is needed to witness the signing, by the Parent or legal guardian, of the passport application. Children 14 years of age or older also sign the application. All applicants under the age of 16 need to be present when visiting the AA.
- AAs are located in Post Offices and Courthouses around the country. Not every Post Office or Courthouse has an AA, and hours vary. Click here to locate your closest AA.
- The AA’s job is to witness the applicant sign the government form, check their ID, and seal some of the application materials into an envelope.
- The AA should write the applicant’s name, departure date, and date of birth on the outside of the sealed envelope. If two or more applications are sealed in the same envelope, ensure that all applicant’s information is written on the outside of the sealed envelope.
- Tell the AA that you are using DC DAATS to expedite the application. They must return the envelope to you. It is occasionally necessary to refer the AA to the Department of State’s “Hand Carrying of Executed Applications” policy, located on page 31 or 44 of their manual. If the agent refuses to return the envelope, call DC DAATS to have a representative assist you.
Step 4 - Ship Documents to DC DAATS
- Be sure that you send the government sealed envelope and the rest of the documents, arranged as listed in step 1. Any discrepancy can delay processing.
- The federal government requires that we submit your application within 5 business days of it having been sealed.
- Include a copy of an unsigned DS-11 form and a completed Letter of Authorization along with the sealed envelope
